What to Expect When You Call
- Your AMS Referral Coordinator will ask you for all the information we need to process the referral
- Patient’s name, address, phone number, date of birth, and Social Security number
- Medical reason for the referral/diagnosis and any testing results
- Specialty requested and, if applicable, specialist requested
- Employer and/or Payer and or Adjuster’s name, contact person, and the contact person’s phone and fax numbers
- Referring physician’s name, address, phone number, NPI number, and, if applicable, contact person
- Your AMS Referral Coordinator will set up your account
- Establish method of billing
- Establish communication guidelines
